Traffic was brought to standstill on a busy road when a a man was beaten to the floor in a three-on-one road rage attack.
The dramatic footage - filmed by a motorist - is believed to have happened near Birmingham City Hospital yesterday.
A minute-long clip shows a man in a white t-shirt sitting on the bonnet of a black Volkswagen to stop it from leaving, as the driver tries to push him out of the way.
The man, who appears to be calling the police, tells the driver: 'You can't go nowhere, you can't go nowhere.'
He replies: 'Yo, just get your face out the car.'
The driver accelerates, causing him to jump off the bonnet then stops several metres away.
His passenger leaps out and storms towards the man, throwing punches at him.
The driver then joins in, swinging wildly and ripping the man's shirt as he bravely tries to fight them off.
Witnesses rush to intervene, shouting: 'Oi. Oi.'
But the vicious attack rages on and a third assailant punches the man in the head from behind, knocking him to the ground.
The thugs run back to their car and speed away down the wrong side of the road.
Witnesses rush to the victim's aid as he collapses to the tarmac.
Facebook users were appalled by the 'cowardly' attack.
Mo Azim said: 'Cowards rushed one man on his own.'
Gavin Dring wrote: 'What a shameful pathetic spectacle. I just wish people like this could be deleted off the planet.'
Callum Hughes added: 'Winds me up, man. You can tell the geezer could probably handle himself but you ain't got no chance if there's 3 geezers coming at you from all directions.'
West Midlands Police has been asked to comment.
